Analysing the corticospinal tract (CST) in patients with parkinsonism is one of the most powerful means of assessing the status of the motor system and the efficacy of therapeutic interventions. Although the traditional method of analysis involves volumetric measurements of the CST, recent technological advances have made it possible to map the whole tract and to perform volumetric analyses of specific structures within it. This study used fibre-tracking techniques and diffusion tensor imaging to explore CST architecture in patients with Parkinson’s disease (PD). An improved protocol using higher field strength (7T) was applied and a novel cortical-based approach was developed, which enabled the evaluation of the whole CST and its major subdivisions in all 12 individuals. Our data showed that the majority of alterations were observed in the inferior part of the anterior limb of the internal capsule (ALIC) in patients with PD, with four sub-regions showing specific abnormalities, namely the middle part, the superior part and the anterior and posterior borders of the ALIC. The latter corresponded to the lateral posterior limb of the ALIC. The volumetric analysis of the CST in patients with PD was also very useful in confirming specific topographical abnormalities in these patients. Furthermore, we showed that the detailed mapping of the cortical territory of the CST provided a more comprehensive means of describing its neuroanatomical changes in PD. Our results, which are in line with previous studies using volumetric techniques, also show that fibre-tracking techniques have more utility in studying cortical connectivity in PD. This study highlights the importance of using multiple techniques to define the extent of CST changes in PD. and there are no witnesses or records from the time period at issue. While this is not a valid reason for lack of notice, it is certainly a factor in determining if a motion to reopen is to be granted. However, in this case, the appellant presented no such evidence.
